Issue when rescheduling

Clicking on the "Confirm" button on the second screen should reschedule the meeting, but instead just reloads the form.

So the function "Offer to reschedule last active booking" is not working.

To replicate: 

1. Create a new form 

2. Enable the option "Offer to reschedule last active booking" 

3. Make a booking and try to reschedule the active booking  BUG: Form can not be submitted
