Add Pull Request Template

Hey team! 

Happy to open a PR to add this, but I didn't want to step on your toes. I see the `CONTRIBUTING.md` doc - is there any work in progress to add a PR template to this repo? I was looking to make a small docs change, and noticed there wasn't one. Not a big deal! Just curious!
